TSF-01 North Tailings Storage Facility — conventional paddock, ACTIVE, HIGH consequence, MOL 248.5 mRL, min freeboard 1.5 m
EMB-N / DEC-01 / SPW-01 Northern embankment, primary decant, emergency spillway (components)
DEMO_EOR Boulder Ridge Geotechnics — Engineer of Record (design basis v2.1, APPROVED)
PZ-N-01 North embankment vibrating-wire piezometer (telemetry) — the TARP trigger source
PZ-N-01-TRIG Threshold set: Level-1 > 130 kPa, Level-2 > 150, Level-3 > 170
TSF-EVT-2026-00042 62 mm rainfall event, 12 Jul 2026 — raised pond level and pore pressures
TSF-TARP-2026-00011 LEVEL_1 activation — PZ-N-01 hit 138.5 kPa; deactivated after 48 h downtrend
TSF-INSP-2026-00713 Daily inspection result DEFECT — new crest crack on EMB-N
TSF-DEF-2026-00019 Defect: ~15 m crest crack — CLOSED after verified repair

TSF-NCR-2025-0007 Stage-3 compaction nonconformance — reworked and CLOSED
TSF-ENV-2026-0056 Downgradient bore MB-04 — groundwater sulfate 640 mg/L exceeds the 500 guideline
TSF-REV-2026-0003 Annual Dam Safety Review 2026 — findings F-01 (monitoring), F-02 (survey)
KPI-INSTR-DATA Instrument data availability KPI — currently WARNING (98.6% vs 99% target)

Status Vocabularies (the important ones)

Facility status: PROPOSED → DESIGN → CONSTRUCTION → COMMISSIONING → ACTIVE → CARE_AND_MAINTENANCE → CLOSURE → POST_CLOSURE → DECOMMISSIONED

TARP level: NORMAL → LEVEL_1 → LEVEL_2 → LEVEL_3 → EMERGENCY — escalation is graded; de-escalation is a recorded, approved decision

Reading status: VALID · ESTIMATED · SUSPECT · REJECTED · MISSING · SUPERSEDED — only VALID/ESTIMATED should update Monitoring Current

Action status (Defect, Corrective Action, Nonconformance, Review Finding, TARP execution): OPEN → ASSIGNED → IN_PROGRESS → BLOCKED → COMPLETE → VERIFIED → CLOSED, plus CANCELLED, OVERDUE

Work order status: REQUESTED → APPROVED → PLANNED → RELEASED → IN_PROGRESS → COMPLETE → VERIFIED → CLOSED, plus CANCELLED

Severity (events, defects, findings, component criticality): INFORMATION · LOW · MODERATE · HIGH · CRITICAL · CATASTROPHIC

Inspection result: SATISFACTORY · OBSERVATION · DEFECT · SIGNIFICANT_DEFECT · UNSAFE · INCOMPLETE

Document status: DRAFT · REVIEW · APPROVED · ACTIVE · SUPERSEDED · WITHDRAWN · ARCHIVED Closure status: NOT_STARTED · PLANNED · IN_PROGRESS · MONITORING · CRITERIA_PARTLY_MET · CRITERIA_MET · ACCEPTED

Rules of Thumb

  • An ACTIVE facility should have an accountable position, one APPROVED design basis, and an ACTIVE TARP plan and emergency plan.
  • Only one threshold set per code should be ACTIVE for a facility at an effective date; a breach opens a TARP activation against the specific rule and the reading/event that triggered it.
  • A TARP activation is not auto-deactivated by a later normal reading, and LEVEL_3 / EMERGENCY de-escalation needs authorised technical approval.
  • A CRITICAL or CATASTROPHIC defect needs an immediate operational restriction or a documented emergency decision.
  • A COMPLETE work order does not close its corrective action or defect — closure requires recorded verification.
  • Overdue inspections, calibrations, reviews and actions stay visible on the operational dashboard.
  • Water-balance variance beyond tolerance needs investigation and approval before the period is approved.
  • A closure criterion reaches CRITERIA_MET only with a current monitoring result and evidence.
